Benevolence Request Form

Christian Life Fellowship understands that life can bring unforeseen hardship and may be able to provide assistance if you are an active CLF Family Member. The following are requirements to receive assistance.

A CLF Family Member is someone who:

1. Regularly attends a weekly service
2. Has completed LIFE TRACK
3. Tithes on a regular basis

The CLF Benevolence Committee will review your responses and, if approved, send you the Financial Assistance Application to continue the process. If this form is denied, the committee will send you a link to our Community Resource page, where you can find many resources to help you within Lee County.
